Prices are based on the set session time, additional time is billed per minute by our hourly rate ($180/hr). This includes but is not limited to: supplementary assessments, form/note completion, and care team consultations outside of the call. We recommend completing and submitting all forms to the best of your ability at least 48 hours prior to your session and connecting on time with minimal distractions to ensure the best use of your time during our assessment. Appointments cancelled due to incomplete forms or failure to provide 48 hours notice of cancellation will not be refunded. ​

NP Jen will not provide standalone forms completion or second opinion diagnosis for patients under active medical management for their condition.
